Transitional Living Case Manager

We are seeking a positive, enthusiastic, multitasker to join our team. The ideal candidate is ambitious and has a passion for helping others. We have an immediate opening for a self-motivated individual to be the next rock star on our Transitional Living Center team. Under the direction, guidance, and supervision of the Program Manager, the Case Manager assumes specific program responsibilities for the Transitional Living Center. These responsibilities include overseeing and documenting the needs of residents through advocacy and connecting them with resources within the community. This requires developing a working relationship with other community agencies and organizations. The Case Manager will work with the resident in developing and following their action plan. Additionally, the Case Manager is responsible for providing on-call services and responding to 24-hour calls of service to TLC residents and staff.

• Provide intensive case management for new residents in the facility and transitioning to lighter case management as needed. This includes working with and referring residents to the community, medical, educational, social service and other agencies to schedule needed services and linkages.
• Complete all required intake documentation and maintain client files.
• Accurately document all services provided in a timely manner
• Attend and participate in monthly file reviews
• Schedule initial physical assessments and other medical appointments as needed for intake requirements for residents and their children.
• Represent the agency and program in community meetings regarding residential care for homeless women as assigned.
• Communicate and coordinate with other community resource providers, mental health providers,
DCFS and program staff regarding case management activities and resident care issues as needed.
• Develop, review and document individualized action plans and make necessary revisions on an on-going basis
• Develop agenda and facilitate bi-monthly group sessions with TLC residents as needed.
• Requires the ability and skills to intervene appropriately in a crisis situation
• Requires the ability and skills to communicate effectively with a diverse population
• Requires the ability and knowledge to teach parenting skills
• The ability to provide individual and group counseling, family crisis intervention, conflict resolution, stress management and other like skills training with aftercare contact and support.
• Develop a working relationship with various social service agencies to ensure that appropriate referral criteria and requirements are established and adhered to for clients

• Must have the ability and skills to communicate effectively with a diverse population
• Requires the ability to maintain professional and courteous composure at all times
• Requires visual ability, auditory ability and ability to speak

The Case Manager will have a minimum of a Bachelor’s Degree in the social services field from an accredited college or university and a minimum of three years of direct service in the human service area.
