Case Manager

Position
We are seeking a positive, enthusiastic, multi-tasker to join our team. The ideal candidate is ambitious and has a passion for helping others. We have an immediate opening for a self-motivated individual to be the next rock star on our Sexual Assault Victim Care Unit. Under the supervision of the Director of the Sexual Assault, the Case Manager’s responsibilities include overseeing and documenting the needs of clients through advocacy and connecting them with resources within the community. This requires developing a working relationship with other community agencies and organizations. The Case Manager will work with the client in developing and following their action plan. Additionally, the Case Manager is responsible for providing on call services on a rotational basis. The Case Manager will offer support and resources to survivors of sexual assault while advocating to end sexual assault and abuse within our community. The Sexual Assault Victims Care Unit is an Illinois Coalition Against Sexual Assault (ICASA) member Rape Crisis Center serving Madison, St. Clair, Monroe and Randolph Counties.

Duties
• Provide intensive case management for survivors/clients and transitioning to lighter case management as needed. This includes working with and referring residents to community, medical, educational, social service and agencies to schedule needed services and linkages
• Complete all required intake documentation and maintain client files
• Accurately document all services provided in a timely manner
• Attend and participate in monthly file reviews
• Represent the agency and department in community meetings regarding sexual assault
• Develop and maintain outreach to children and adolescents by contacting and coordinating with schools
• Communicate and coordinate with other community resource providers regarding case management activities as needed
• Develop, review and document individualized action plans and make necessary revisions on an ongoing basis
• Participate in staff meetings and staff development
• Provide individual supportive guidance on a weekly basis or as needed
• Assist survivors/clients in coordinating and scheduling appointments; providing transportation when needed
• Report all findings of abuse/neglect to the immediate supervisor as a mandated reporter
• Develop a working relationship with various social service agencies to ensure that appropriate referral criteria and requirements are established and adhered to for clients
• Provide on-call advocacy on a rotational basis
• Requires the ability to work necessary hours to meet the position requirements including evenings and/or weekends as assigned
• The omission of specific functions does not preclude management from assigning other duties when the Board of Directors believes it to be in the best interest of the program and Call for Help
• Meet monthly with the Program Director for supervision regarding the residents and program unless specified otherwise.

• Must maintain confidentiality at all times
• Requires effective time management to meet position, report and assignment needs
• Requires the ability and skills to intervene appropriately in a crisis situation
• Must have the ability and skills to communicate effectively with a diverse population
• The ability to provide individual and group counseling, family crisis intervention, conflict resolution, stress management and other life skills training with aftercare contact and support

Qualifications
The Case Manager will have a minimum of a Bachelor’s Degree in the social services field from an accredited college or university and a minimum of three years of direct service in the human service area.
